The Program
The In Home and Family Support Program was developed in response to family member requests
for additional support in caring for their spouse, child, parent, or friend.
With services that focus on supportive counseling, coordination of needed services, and advocacy, the goal
of this program is to assist individuals in achieving their maximum level of independence while living
in the community of their choice. At their own pace, participants are assisted in identifying their
individual needs and locating local support resources within the community.
Ultimately, individuals are aided in maintaining emotional health, well being, and continued recovery.
Support Services
Each individual is assigned a personal Counselor. Regularly scheduled, ongoing contacts are designed to meet
the needs of the client. Weekly meetings are utilized to provide consistent and continuous support and guidance.
As the client stabilizes and becomes more independent, meetings are modified to address any continuing needs.
With the assistance of their Counselor, participants are supported in activities of daily living with a focus
on identified areas of rehabilitation such as:
Assertiveness/Self-Advocacy
Community Integration
Daily Living Skills
Health Services
Medication Management
Parenting Skills Training
Rehabilitation Counseling
Skill Development Services
Socialization Assistance
Substance Abuse Relapse Prevention
Symptom Management
With the participant's permission
collaborative meetings are arranged with family members, therapists, psychiatrists, day programs, and
employers to facilitate cohensive care.
24-Hour Emergency Services
During instances of clinical and/or medical emergencies, crisis
intervention services requiring the assistance of our staff are available 24-hours per day.
Monthly Support Meetings
Members are encouraged to participate in monthly group dinners and meetings to facilitate
the development of a personal support network.
Recreational/Social Events
Based on individual interest and involvement, program-sponsored recreational and social
events are available to all program participants, encouraging socialization and solidification
of personal support networks.
Quality Assurance
Consumer Satisfaction Surveys are issued twice annually
because consumer input is so integral to the success and integrity of SfC's services.
The confidential survey results
are compiled and used for quality assurance analyses, to facilitate program improvements,
and in the formulation of future service initiatives.
