Supported Housing offers individuals an opportunity to exercise
personal
choice in selecting where they would like to live. Participants
are
assisted in securing available housing within the community and
with
procuring initial furnishings.
After transitioning to their new apartment, in-home support and
assistance
continues until the individual feels secure that they can comfortably
maintain themselves in their new environment. For some individuals,
support can extend for a brief period of six months. For others,
services
can be utilized indefinitely.
Supported Housing Program staff maintain contact with participants
through monthly support group dinners, weekend social activities, bi-monthly
shopping trips, and annual vacations. Staff also provide assistance
with
daily living tasks, along with any necessary advocacy services to
coordinate
treatment, rehabilitation, benefits, and general community integration.
Although candidates for this program must apply and ultimately qualify
for
Section 8 subsidies, interim rent stipends may be available to ensure
individuals pay no more than 30 % of their monthly income for rent.
This
program is funded through reinvestment grants, county resources
and the
assistance of the Office of Mental Health. The grants provided allow
the
agency to aid individuals with rent stipends, apartment set-up costs/needs,
and ongoing maintenance.
